Job purpose
The Clinic Nurse assists with the coordination of the psychiatrist's/nurse practitioner's daily activities. The Clinic Nurse also plans and manages patient care according to each patient's needs.
Qualifications
- Licensed Practical Nurse (LPN) with Two (2) years of supervised related work experience.
- Psychiatric experience and experience with healthcare technology preferred.
- Must be capable of accurately recording and communicating diverse information and monitor said information;
- Meet the minimum requirements to be clinically privileged by FCC Behavioral Health.
Core Job Duties
- Evaluate person(s)-served physical condition.
- Obtain initial patient history.
- Take person(s)-served vital signs.
- Maintains timely, accurate, and well-written documentation associated with all person-served events.
- Maintain a monthly schedule for injections and administer injections.
- Perform screenings per Prescriber's request.
- Request lab results and follow up with reported results.
- Call in prescriptions and refill medications as needed and directed by the Prescriber.
- Enter all controlled medications and new medications.
- Manage Patient Assistance for all medications for the Prescribers.
- Complete prior authorizations for Medicare/Medicaid/insurances for the Prescribers.
- Answer phone calls regarding medications and notify Prescriber of adverse reactions.
- Weekly updates on sample medication inventory and storage as well as quarterly inventory report.
- Keep track of sample medication inventory and storage, as well as quarterly inventory reports.
- Manage the billing for Telehealth services.
- Handle request for records/discharge summaries as requested by Prescriber.
- Triage any medical conditions that occur during treatment.
- Participate in clinical staffing of all person(s)-served with supervisor and treatment team. Provide feedback to the treatment team in regard to medically related issues.
- Effectively manage any medical emergencies that may arise.
- When applicable, educate person(s)-served and family members (as necessary) about medication(s) prescribed to person(s)-served as well as side effects.
- Monitor medication compliance.
- Maintain inventory of all medical supplies.
- Ensure physician orders are appropriately followed.
- Accurate and timely completion of all medical interventions provided to person(s)-served.
- Ensuring communication and coordination with and between multiple service providers.
- Run EmoMed reports on person(s)-served on a regular basis and keep Supervisor updated.
- Ability to effectively utilize computer systems currently employed by FCC.
- Maintain/improve professional competencies through training activities and/or other educational means.
- Maintain awareness regarding the agency vision and assist staff in continuing motivation and empowerment.
- Participates in clinical staffing of person(s)-served to include attendance of meetings and/or written notes regarding person(s)-served cooperation and participation with program expectations.
- Complete all required medication documentation in record of person(s)-served.
- Treatment plan review forms completed accurately and in a timely manner.
- Accurate and timely completion of group counseling and group education logs, as required.
- Accurate completion of all required program specific medication forms.
- Accurate documentation of person(s)-served medical history and nursing assessment.
- Document specific aftercare plans in regard to medical follow-up.
- Consistently complies with supervisory directives as it applies to enforcement of facility policies and procedures with person(s)-served.
- Maintain/improve professional competencies through training activities and/or other educational means.
- Demonstrate a working knowledge of diagnostic criteria sets for substance use and/or mental disorders.
- Establish and maintain effective working relationships with individuals at all levels of the organization.
- Listen to others to facilitate an open exchange of ideas and provide for effective communication.
- Orally communicate information effectively and accurately.
- Assure program operates within the constraints of the agency's certification (CARF, DMH) standards and strategic plan related to clinical care.
- Maintain confidentiality, at all times, within the legal framework. Will adhere to established guidelines regarding confidentiality as well as the agency's adopted code of ethics.
- Maintain accurate and up-to-date records of person(s)-served as outlined in the agency and site Policies and Procedure Manuals.
- Maintain acceptable attendance record, respect break periods and work with supervisor to ensure coverage during any absence.
- Exhibit a professional appearance, attitude and demeanor that reflects confidence, positive self-esteem and overall professionalism when working with person(s)-served, co-workers, families and referral sources.
