Assure agency programs operate within the constraints of the agency's certification (CARF, DBH) standards and strategic plan related to clinical care with periodic site reviews that focus on general clinical standards of care.

Assists the person(s)-served and Psychiatrist in the development and maintenance of an individualized care plan prior to the tenth (10) session and monitors progress toward outcomes specified in the plan.

Maintains confidentiality at all times within the legal framework.

Identify person(s)-served who receive Medication Services only.

 Will adhere to the licensing board's guidelines regarding confidentiality, as well as the agency's adopted Code of Ethics.

 Assist person(s)-served with gaining access to medical, social, educational, and/or therapeutic services and supports recommended by the Psychiatrist.

 Will follow up with all no-shows by telephone or individualized letter, and will document at least two (2) attempts to re-engage person(s)-served within 72 hours of missed appointment.

Identify individuals who are no longer participating in medication services for discharge from the agency and complete required discharge paperwork.

 Participate in discharge planning with the Psychiatrist and person(s)-served. Is accepting of individual differences among person(s)-served.

 Will formally present cases in clinical staffing as assigned. Attend clinical staffing as scheduled to staff referrals for additional services and to participate in staffing referrals for Physician's Case Management services

Demonstrates a caring and compassionate approach to person(s)-served care. Identify person(s)-served who receive Medication Services only.

Will complete all care plans and clinical progress notes in a timely fashion. I

Interacts with peers in a manner that promotes cohesiveness and support.

Interacts professionally and appropriately to all agency staff. Exhibits a professional appearance, attitude and demeanor that reflects confidence, positive self-esteem, and overall professionalism when working with person(s)-served, co-workers, families and referral sources.

Demonstrates initiative and a "can do" attitude.

Is oriented to problem-solving methods when conflicts arise.

Other job duties and special projects as assigned.

Provide weekly reports in staffing/supervision about new admissions and individuals receiving services that require team advice.

Treatment plan review forms completed accurately and in a timely manner.

Progress notes written accurately and in compliance with the Rule of 72. Discharge/Transition/Aftercare Summary completed accurately and in a timely manner.

Accurate and timely completion of group counseling and group education logs.

Timely completion and submission of all payroll documents.

Timely completion and submission of staff reimbursement forms.

Timely completion and submission of credit card transaction envelope.
