This position can be held by a LPN/RN.

Champions healthy lifestyle changes, including tobacco cessation and prevention, nutrition, physical activity and substance abuse prevention.

 Assists HCH Director to develop and enhance wellness initiatives, health education groups and activities for person(s)-served, staff and community.

 Coordinates care with external healthcare providers, including MO HealthNet contracted pharmacies and providers.

Identifies high risk patients through annual health screening and assessment with person(s)-served and makes appropriate referrals, if needed.

Receives, identifies and follows-up treatment and medication alerts reported by HIT tools and communicates algorithm concerns, problems or suggestions to the HCH Director Arranges for and ensures access to primary care and other healthcare services.

Possesses medical assessment skills, knowledge of medical terminology, diagnoses and medications

Schedules appointments for person(s)-served, as needed.

Monitors critical chronic disease indicators.

 Consults with Care Coordinators about identified health conditions of person(s)-served and provides educational training on chronic disease states, health coaching, medications and healthy living.

Tracks that all required assessments and screenings, including a health screening and metabolic screening are complete for each person-served on their caseload.

Reviews and signs off on the health screening with person(s)-served and identifies health and wellness needs and goals prior to the annual assessment.

Ability to orally communicate information effectively and accurately.

Assure program operates within the constraints of the agency's certification (CARF, DMH) standards and strategic plan related to clinical care.

Participates in monthly HCH team meetings with the HCH Physician Consultant, HCH Director and others as appropriate.

Ability to establish and maintain effective working relationships with others.

Ability to interact with and relate effectively to individuals at all levels of an organization.

Ability to listen to others to facilitate an open exchange of ideas and provide for effective communication.

Maintain's confidentiality at all times within the legal framework.

Will adhere to established guidelines regarding confidentiality as well as the agency's adopted code of ethics.

Maintain's acceptable attendance record, respect break periods and work with supervisor to ensure coverage during any absence.

Exhibits a professional appearance, attitude and demeanor that reflects confidence, positive self-esteem and overall professionalism when working with person(s)-served, co-workers, families and referral sources.

Demonstrates initiative and a "can do" attitude.

Attends state required meeting and trainings.

Other job duties and special projects as assigned.
