Job purpose 

The Care Coordinator (CC) Supervisor will promote FCC's vision of Person-Centered Recovery and Wellness. CC Supervisor's provides supervision to ensure that CC services focus on helping individuals and family develop skills, access resources and learn to manage illness in order to be successful in the living, working, learning and social environments of their choice. 


  • Master's degree in social work, psychology or related field, AND
  • Prior supervised related work experience, preferred.
  • Meet the minimum requirements to be clinically privileged by FCC Behavioral Health.

Core Job Duties 

  • Provide clinical supervision to Care Coordinator's.
  • Complete intakes, assessments and develop treatment plans.
  • Review charts for program, HIPPA and corporate compliance.
  • Provide orientation to new Care Coordinator's.
  • Provide crisis intervention to person(s)-served during office hours.
  • Evaluate staff performance and complete staff development plan.
  • Initiate staff incident report when necessary and create an action/development plan for Care Coordinator's.
  • Facilitate staff meetings.
  • Collect itineraries from Care Coordinator's that do not have GPS on vehicle and complete monthly form.
  • Run a weekly Black Box Report and compare to one service delivered for the week on CC's that have a GPS in their company vehicle.
  • Conduct quarterly peer audit reviews with the Supervisory Team at all locations.
  • Ensure completion of the bi-annual satisfaction surveys for person(s)-served at all clinical sites.
  • Report staffing recommendations to the Clinical Manager.
  • Assure written reports to appropriate person(s) and/or agencies are completed in a timely manner, including the provision of such when necessary.
  • Helping individuals develop skills and resources that foster success at work.
  • Ensuring staff under direct supervision are communicating and coordinating with and between multiple service providers.
  • Assure individualized recovery care plans for person(s)-served are developed with SMART goals, objectives and interventions.
  • Assure treatment plan review forms are completed accurately and in a timely manner.
  • Assure staff progress notes are written accurately using the PIRP format and in compliance with the 5-Day Rule (and/or site policy)
  • Assure staff are providing quality documentation to fully support all Community Support billing.
  • Assure staff maintain a minimum productivity rating of 60% of all available work hours to ensure access to clinical care.
  • Assure Discharge/Transition/Aftercare Summaries are completed accurately and in a timely manner.
  • Maintain/improve professional competencies through training activities and/or other educational means.
  • Maintain awareness regarding the agency vision and assist staff in continuing motivation and empowerment.
  • Demonstrate a working knowledge of the DSM and familiarity with the diagnostic criteria sets of all substance use and mental disorders.
  • Participates in clinical staffing of person(s)-served to include attendance of meetings and/or written notes regarding person(s)-served cooperation and participation with program expectations.
  • Consistently complies with supervisory directives as it applies to enforcement of facility policies and procedures with person(s)-served.
  • Ability to effectively utilize computer systems currently employed by FCC.
  • Maintain/improve professional competencies through training activities and/or other educational means.
  • Demonstrate a working knowledge of diagnostic criteria sets for substance use and/or mental disorders.
  • Establish and maintain effective working relationships with individuals at all levels of the organization.
  • Listen to others to facilitate an open exchange of ideas and provide for effective communication.
  • Orally communicate information effectively and accurately.
  • Assure program operates within the constraints of the agency's certification (CARF, DMH) standards and strategic plan related to clinical care.
  • Maintain confidentiality, at all times, within the legal framework. Will adhere to established guidelines regarding confidentiality as well as the agency's adopted code of ethics.
  • Maintain accurate and up-to-date records of person(s)-served as outlined in the agency and site Policies and Procedure Manuals.
  • Maintain acceptable attendance record, respect break periods and work with supervisor to ensure coverage during any absence.
  • Exhibit a professional appearance, attitude and demeanor that reflects confidence, positive self-esteem and overall professionalism when working with person(s)-served, co-workers, families and referral sources.
  • Demonstrate initiative and a "can do" attitude.
