Summary of Position 

The services provided by the Care Coordinator (CC) focus on helping individuals and family develop skills, access resources and learn to manage illness in order to be successful in the living, working, learning and social environments of their choice. CC's teach, model and practice skills with person(s)-served in order to promote recovery and wellness. 

Qualifications                       

  • Bachelor's degree in social work, psychology or related field, OR four (4) years of supervised related work experience, OR four (4) years combination of education and experience
  • Meet the minimum requirements to be clinically privileged by FCC Behavioral Health. 

CARE COORDINATOR LEVEL II 

  • Bachelor's Degree in Psychology, Social Work, Criminal Justice and/or other Social Service related field, OR
  • 10,000 hours (at least five (5) year) of total applicable work experience, OR
  • Combination of education and applicable work experience equaling up to five (5) years. 

CARE COORDINATOR LEVEL III 

  • Master's Degree in Psychology, Social Work, Criminal Justice and/or other Social Service related field, OR
  • 14,000 hours (at least seven (7) years) of total applicable work experience, OR
  • Combination education and work experience equaling up seven (7) years.                                                                                                                                                                                                                               

Core Job Duties 

  • Use interventions, to develop interpersonal/social, family, community and independent living functional skills including adaption to home, school, family and work environments when the acquisition of these skills is impacted negatively by the individual's health, mental illness and/or substance use.
  • Provide information/education in order to learn about and manage health, mental illness and/or substance use disorders including symptoms, triggers and cravings and reinforce the importance of taking medications as prescribed, while facilitating the person(s)-served communication with prescribers as needed.
  • In conjunction with the individual, family and/or significant other, identifying risk factors related to relapse in health, mental illness and/or substance use disorders and develop a plan with strategies to support recovery and prevent relapse.
  • Ensure individuals gain access to necessary rehabilitation services, general entitlement benefits, housing, schools, employment, legal services, wellness or other services and gain skills in independently accessing needed services.
  • Assist person(s)-served in the development, maintenance and achievement of goals/objectives on their care plan.
  • Assist person(s)-served in the development, maintenance, wellness and/or independent living skills such as: housekeeping, cooking, personal hygiene, shopping, laundry, budgeting and doctor's appointments.
  • Developing/supporting recovery and wellness goals in collaboration with the individual and family including healthy lifestyle changes such as healthy eating, physical activity and tobacco prevention and cessation.
  • Provide interpersonal support/guidance to assist person(s)-served in problem solving and decision making.
  • Provide holistic, person-centered care with emphasis on harm reduction and use of motivational approaches that promote active participation in decision making and self-advocacy in all aspects of services and recovery/resiliency.
  • Building skills for effective illness self-management including psycho-education, behavioral tailoring for medication adherence, relapse prevention training, coping skills training and social skills training.
  • Developing an aftercare recovery plan to include fostering a successful transition to continued services.
  • Ensure follow-up with recommended medical care, to include scheduling appointments, finding financial resources and arranging transportation when individuals are unable to perform these tasks independently.
  • Contacting individuals in crisis situations including locating and coordinating resources to resolve a crisis.
  • Maintain contact with individuals who are hospitalized and participate in discharge planning.
  • Helping individuals develop skills and resources that foster success at work.
  • Ensuring communication and coordination with and between multiple service providers.
  • Ability to effectively utilize computer systems currently employed by FCC.
  • Maintain/improve professional competencies through training activities and/or other educational means.
  • Maintain awareness regarding the agency vision and assist staff in continuing motivation and empowerment.
  • Demonstrate a working knowledge of the DSM and familiarity with the diagnostic criteria sets of all substance use and mental disorders.
  • Participates in clinical staffing of person(s)-served to include attendance of meetings and/or written notes regarding person(s)-served cooperation and participation with program expectations.
  • Consistently complies with supervisory directives as it applies to enforcement of facility policies and procedures with person(s)-served.
  • Establish and maintain effective working relationships with individuals at all levels of the organization.
  • Listen to others to facilitate an open exchange of ideas and provide for effective communication.
  • Orally communicate information effectively and accurately.
  • Assure services are being provided within the constraints of the agency's certification (CARF, DMH) standards and strategic plan related to clinical care.
  • Maintain confidentiality, at all times, within the legal framework. Will adhere to established guidelines regarding confidentiality as well as the agency's adopted code of ethics.
  • Maintain accurate and up-to-date records of person(s)-served as outlined in the agency and site Policies and Procedure Manuals.
  • Maintain acceptable attendance record, respect break periods and work with supervisor to ensure coverage during any absence.
  • Exhibit a professional appearance, attitude and demeanor that reflects confidence, positive self-esteem and overall professionalism when working with person(s)-served, co-workers, families and referral sources.
  • Demonstrate initiative and a "can do" attitude.
